What is an Assessment

Reaching out for an assessment is a big step, and we’re glad you’re here. An assessment is a private, judgment-free conversation with one of our licensed therapists. It helps us understand what you’re going through so we can recommend the care that fits you best.

What to expect

Your therapist will talk with you about your history with alcohol and drugs, how substance use is affecting your life right now, and your mental and physical health. There are no wrong answers. This is simply a chance for you to share your story and for us to listen.
The assessment usually takes 1 to 2 hours. By the end, you’ll have a clear picture of where you stand and a personalized recommendation for next steps, built around your goals, needs, and life.

When we’re available

  • Monday through Friday, 8:00 am to 4:00 pm
  • Evening and weekend appointments available by request

Yes. Every assessment is conducted in a private setting, and your personal information is handled in accordance with all applicable privacy regulations.
No. You can contact Asurgent Health directly to schedule an assessment without a physician’s referral.
Please bring a valid photo ID, insurance information (if applicable), a list of current medications, and any relevant medical records if available.
